Wilmslow RUFC Mini/Junior Registration – Sunday 7 September 2008

The following is a summary of what is required from all the age groups on what is usually one of the busiest Sundays of the season. It is not prescriptive but merely a guide to help the younger age group coaches/parent helpers. Please note children under the age of six cannot play representative games (See the sections of the Continuum dealing with very young players).
If in doubt seek advice from Chairman of the Mini/Junior section,
Martin Hill or Kit Clucas, Club Membership Officer.

New Coaches ‘Blue’ Form & CRB Form

-Obviously all new coaches, First Aiders & Parent Helpers, must complete a CRB form. Please see Anne Smith CPO or Kit Clucas, Membership Secretary for one of these.-Also a ‘Blue’ form needs completing. These capture basic information on the coaches, & were originally printed on blue paper. Kit will supply each coach with a couple of spare forms.

Membership Forms:

Existing Players:

All mini/junior players that were on the registration database at the end of the season should have received, over the summer, a mailing with a renewal application form. This form had all the relevant information we held inserted into the form. The parent/guardian of the child was asked to check the details and amend any that were incorrect or add any missing and either bring to registration or preferably post in advance with their payment. Details of what should be added to the form for our benefit are explained later in this note.
Any existing member, who has either lost or not received the form, can complete a general form, used for new players, and there will be adequate copies on hand on Registration day.

New Players:

All new players will have 2 forms to complete, a WRFC club form & an RFU ‘Young Player Registration Form’. This is required to obtain an RFU registration number. The parent/guardian must sign both forms and all sections should be completed if applicable. We will require two passport size photos.
The WRFC form can be downloaded from the website. All coaches will be given a supply of these at Registration.

Finally if you are asked for Social, Senior or Retired membership then a senior membership form should be completed.

If in any doubt please contact Martin Hill or Kit Clucas on Registration Sunday.

Details we need on form:

To help with the registration database we need the following information on the form:- Please put this information in the top right hand corner of the form:
-1 Whether paid or trial (only for new players and at the younger age groups) .

2 Type of payment (cash/cheque) & amount if applicable.

3 If you receive a cheque to cover more than one membership please cross-refer on each of the forms e.g. Liam & Michael Heaney are both junior members and each have a pre-filled application form. As well as payment details of “cheque £x” you should add “see also Liam Heaney UXX” on Michael’s and on Liam’s “see also Michael Heaney UYY”.

Forms:

All forms should be handed to Kit Clucas, or in his absence Martin Hill. Kit will scan the forms, produce copies for the coaches and give out handbooks (which double up as membership cards) for those who have paid as well as updating the database.

These forms have medical details & Parent’s consent for medical treatment, & must be kept by the appointed coach at pitch side.

Subscriptions:

This season, we have revised the Membership fees, to include Social Membership for one Parent per player. The costs are as follows:
-Parent / Guardian & 1 Child Membership £65
-Parent / Guardian & 2 Child Memberships £95

-Parent / Guardian & 3 Child Memberships £125
